Youth Flag Football Registration Agreement & Waiver
Parents and legal guardians should understand the participation terms they accept when registering a player.
Participation
Permission to participate
Registration confirms that the parent or legal guardian authorizes the player to participate in the applicable United Sports or NFL FLAG program, including practices, games, camps, clinics, showcases, and tournaments when applicable.
Risk & responsibility
Acknowledgement of sports related risk
Flag football is an athletic activity and includes risks associated with running, contact, falls, weather, field conditions, and participation with other athletes. Registration includes acknowledgement of those risks and the responsibilities that come with participation.
Media
Photography and video
Registration terms may include permission for photography or video captured during United Sports activities to be used for league communication, promotion, social media, and program related purposes, subject to the terms presented during registration.
Emergency care
Medical treatment authorization
Registration terms may also include authorization for emergency medical care when a parent or guardian cannot be reached and immediate treatment is reasonably necessary.
Final agreement
Review the terms during registration
The agreement presented during registration is the agreement you accept for that program. Read it carefully before completing registration, especially any liability release, waiver, consent, media, and emergency treatment provisions.
